WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing today a voluntary recall by Hill Sportswear Inc., of Paramount, Calif., of 300,000 hooded drawstring sweatshirts. CPSC received one report of a death involving a 3-year-old boy in Fresno, Calif. He was strangled when the drawstring on the hooded sweatshirt that he was wearing became stuck on a play ground set.

The sweatshirts, Kid Pullover Hood Sweatshirt and Kid Zipper Sweatshirt with Hood, have drawstrings through the hood. The drawstrings should be removed immediately since they can become caught on objects and strangle a child. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.

The recall includes the Kid Pullover Hood and Kid Zipper with Hood. The Kid Pullover Hood has a flat style drawstring at the neck, one middle front pocket, and fleece inside lining. Top sewn in label reads “HILL/ Made in USA.” The hooded Zipper children’s sweatshirt has a rounded style drawstring at the neck, two front pockets, and fleece inside lining. Top sewn in label reads “HILL/ Made in USA”. The recalled sweatshirts were sold at various small retailers in California and Texas from August 1999 through December 2008 for approximately $8.

Due to the serious nature of this hazard, parents are urged to immediately remove the drawstrings from the sweatshirts or return the garment to either the place of purchase or to Hill Sportswear for a full refund.

Incidents/Injuries: The firm has received three reports of detached eyes, including two reports of children who started to choke on the pieces. No injuries have been reported.

Description: The recalled Playhouse Disney “Handy Manny” Tool Set is a plastic, eight-piece toy set that includes a tool box, hammer, saw, pliers, wrench, tape measure and two screwdrivers. The name “Handy Manny” is embossed on one side of the tool box. The UPC (#405144100614 or #422147400633) can be located on the product’s packaging.

Sold exclusively at: The Disney Store nationwide from October 2007 through January 2009 for about $15.

Units: About 213,000

Manufacturer: Evenflo Co. Inc., of Miamisburg, Ohio

Hazard: When used as an activity table, the cap on one end of the product can loosen and fall off, posing a fall hazard to a young child.

Incidents/Injuries: Evenflo has received 11 reports of dislodged end caps which resulted in nine minor injuries, including bumps and bruises. Evenflo has also received a report of a dislodged end cap that resulted in a child suffering a broken collarbone in Canada.

Description: The recall involves Evenflo ExerSaucer Triple Fun stationary activity centers made between October 2006 and December 2008. The model number is 6231711. Stage 3 position involves converting the product into an activity table. No other ExerSaucer models use end caps and, therefore, no other models are included in this recall.

Sold at: Juvenile product and mass merchandise stores nationwide, including Toys ‘R Us and Burlington Baby Depot, from October 2006 through February 2009 for about $120.

Remedy: : Consumers should immediately stop using the activity table in Stage 3 and contact Evenflo to receive a free replacement end cap. The product may continue to be used in Stages 1 and 2 without changes.

Hazard: The lettering “HIGH SCHOOL MUSICAL” with glitter on the pouch used with the manicure kit contains excess levels of lead. Lead can be toxic if ingested by young children and cause adverse health effects.

Description: This recall involves the pouch that contains the “High School Musical” manicure kit. The 7-1/2 inch by 4-3/4 inch plastic see-through pouch has a pink zipper with “High School Musical” printed in yellow gold with glitter and “Sharpay” printed in dark red on one side.

Sold by: LTD Commodities and ABC distributing catalogs and Web Sites and on www.lakeside.com from September 2008 to November 2008 for about $11.

Hazard: A child’s head could get entrapped in the canopy’s window openings, posing entrapment and strangulation hazards.

Incidents/Injuries: The firm has received two reports of a child’s head becoming caught in the canopy’s window openings. No injuries have been reported.

Description: The recalled canopies involve the “Home Sweet Playhome” model made of 100% cotton gingham fabric draped from a bamboo hoop which is suspended from the ceiling to create a cylindrical play space measuring 36 inches wide and 9 feet tall. The canopies have an open bottom, a “door” and three multi-paned “windows.” They were sold in blue and pink. “Home Sweet Playhome” canopies sold after October 2005 are not included in this recall. “The Land of Nod” is printed on a woven label on the canopy.

Sold at: The Land of Nod catalog, Web site www.landofnod.com, and at The Land of Nod stores in Illinois and Washington state from September 2003 through October 2005 for about $100 for the canopy and $139 for the canopy and cushion set.

Remedy: Consumers should immediately stop using the canopies and take them away from children. Contact the Land of Nod to receive a free replacement fabric portion of the canopy and a $15 gift card.

Hazard: Pressure from the buildup of carbon dioxide in the jars of Bath Bombs/Balls or Bath Fizzies can cause the caps on the jars to blow off, posing explosion and projectile hazards. The mixtures also can contain citric acid, which can get into the eyes during an explosion, posing a risk of eye irritation.

Incidents/Injuries: JAKKS and CPSC have received 88 reports of exploding jars, including 13 injuries. Injuries to children aged 6 to 11 years old include three cases of irritated eyes, two eye injuries from flying caps, a cheek welt and a cheek cut. Injuries to adults include four reports of bruising, one swollen joint, and one face gash.

Description: This recall involves the purple caps on jars in the Spa Factory(tm) Spa Fantasy Aromatherapy Fountain and the Bath Benefits(tm) Kit toys. The jars are used to make Bath Bombs/Balls and Bath Fizzies. The following models are included in this recall. The item number is located on the bottom of the product’s packaging.

Description: This recall involves a necklace with a “skull-and-crossbones” symbol and a metal clasp. The skull has eleven pieces of rhinestones on its forehead and one rhinestone on both ends of the crossbones.

Name of Product: Jardine Cribs

Units: About 56,450 (320,000 units were previously recalled in June 2008)

Manufacturer: Jardine Enterprises, of Taipei, Taiwan

Hazard: The wooden crib slats can break, creating a gap, which can pose an entrapment and strangulation hazard to infants and toddlers.

Incidents/Injuries: CPSC has received 19 additional incidents of crib slats breaking. In nine of these incidents, consumers reported that their infant or toddler broke the slat while in the crib. In addition, a 22-month-old child fell through the gap between the crib slats when a slat broke. No injuries have been reported.

Description: This recall involves three models of Jardine wooden cribs with the date codes identified below. Cribs with other date codes are not affected by this recall. The date code and model number are printed on the label located on the inside of the bottom rail of the headboard or footboard.
